• All PIAA paperwork & Permission To Treat Form must be submitted to the Athletic Trainers by August 4, 2025 (1 week before the start of tryouts).
• PIAA paperwork includes Sections 1-6 of the PIAA packet:
  • Sections 1-5 will be completed by answering the questions on the Arbiter Sports website.
  • Section 6 will be uploaded as a photocopy to the Arbiter Sports website.
• All Sections 1-5 must be signed by BOTH student & parent/guardian.
• All Section 6s must be dated ON or AFTER May 1, 2025.
  • Any Section 6 dated PRIOR to May 1, 2025 will NOT be accepted.
• ALL Section 6s must be signed by an authorized medical examiner as indicated at the bottom of the Section 6 (MD, DO, PAC, CRNP, SNP).
  • Any Section 6 not signed will NOT be accepted.
     

**It is HIGHLY recommended all student athletes keep a copy of ALL of their documents at home with them throughout the school year in the event that a document may be misplaced or lost.**

• All PIAA paperwork & Permission To Treat Form must be submitted to the Athletic Trainers by November 10, 2025 for the Winter season, and February 23, 2026 for the Spring season.
  • These dates are 1 week prior to the start of tryouts.
• Any athlete who has ALREADY PARTICIPATED in a sport this academic year does NOT need to submit PIAA Sections 1-6.
• Athletes who have ALREADY PARTICIPATED in a sport this academic year must submit Section 7 of the PIAA forms.
  • This will be uploaded as a photocopy to Arbiter Sports website.
• Athletes who have NOT PARTICIPATED in a sport yet this academic year should follow the instructions above for “Fall Sports Eligibility”.
• Permission To Treat Forms must be completed and uploaded for EVERY SEASON regardless of if you have participated in a previous one that academic year.

We have simplified the registration process, and all waivers/forms are now signed digitally through Arbiter Sports!

Steps to Create an Arbiter Registration Account & Complete Registration:

• Create an Arbiter Registration account (you only need one account for your whole family).

• You may create your account by clicking here.

• Activate your Arbiter Registration account, from the activation link that will be automatically sent to your email. (Note: this email may go into your junk folder).

• If you are having trouble creating your account, receiving the activation email, or logging into it, you may contact support at registration@arbitersports.com or call 800-311-4060.

• Once you have successfully created your account and are logged in, please click here to complete your registration.

• Once you have completed your registration, you will receive an email notification to let you know we have received your registration. Students MUST be registered and cleared to be eligible for participation.

Using Arbiter Sports for the FIRST TIME:

• First create an account using the instructions in the drop down above.

• When you reach the Radnor Athletics landing page please select which sport you are participating in for this season.

• Answer the corresponding questions under each section:
  • The “Participant Information” section is where you will submit the photocopy of Section 6, Section 7, or Section 8
    • Section 6 ONLY is required for all athletes who are participating in their first sport of this academic year.
    • Section 7 ONLY is required for all athletes who have already participated in a sport this academic year.
    • Section 8 (in addition to Section 7) is required for athletes who have answered yes to any of the questions on Section 7. A doctor’s note will be accepted as well.
• The “Agreements” section is where you can sign off on all the agreements pertaining to PIAA participation.
• The final “NCSA College Recruiting “ section can be disregarded unless your child is interested in this. This is not mandatory.

Using Arbiter Sports for the SECOND TIME:

• Log in to your account here (You do not need to create a new registration account. You will use the one you previously made.)

• You will select the correct sport that you wish to participate in this season.

• The information for the following sections should auto-populate. Double check to ensure that all of the information is correct.

  • You will submit the correct corresponding photocopies of Sections 7 or 8.

  • You will need to resign the CIPPE Agreements.

  • The final “NCSA College Recruiting “ section can be disregarded unless your child is interested in this. This is not mandatory.

**It is the student-athlete’s responsibility to ensure they get their paperwork submitted on time. A missed opportunity to try out may occur, if a student fails to meet the deadlines and requirements outlined above.**

Where do I find the PIAA forms and Permission To Treat form?

Why do I need a new physical?

• All CIPPEE physicals (PIAA Section 6) must be dated ON or AFTER May 1, 2024 for the 2024-2025 school year as mandated by Pennsylvania Interscholastic Athletic Association.
   

Who signs off on my CIPPE Physical (PIAA Section 6)?

• An authorized medical examiner (MD, DO, PAC, CRNP, SNP).

• Take a hard copy of your Section 6 with you to your physical appointment to have it filled out and signed off by the medical examiner.
   

What forms do I need to submit now with the new system?

• PIAA Sections 1-5 will be completed by answering the required questions while on the Arbiter Sports Page.

• Sections 6 & 7 will be submitted as a photocopy while on the Arbiter Sports Page.

• Permission To Treat Form will be submitted as a photocopy while on the Arbiter Sports Page.
